In addition, we also have an outpatient rehabilitation physical therapy clinic providing orthopedic, sports medicine, and neurological rehabilitation.

Compensation:
$41.60/hr - $51.16/hr depending on experience and skills.

ABOUT THE POSITION

We are offering home health care and hospice in the patient’s homes throughout all of Curry County, Oregon. We also provide an outpatient clinic where we deliver occupational, physical and speech therapy.

The Occupational Therapist (OT) provides direct rehabilitation services across both outpatient clinic and home health settings. The OT evaluates, plans, and implements evidence-based occupational therapy programs to improve functional independence, safety, and quality of life for patients referred by physicians.

Your responsibilities will include at least:

  • Conduct comprehensive evaluations in both outpatient and home health settings to determine functional deficits and develop individualized treatment plans.
  • Establish, assess, and modify realistic, measurable, and functional goals for diverse patient populations.
  • Provide direct occupational therapy interventions, including ADL training, adaptive equipment education, upper extremity rehabilitation, home safety assessments, and functional mobility training.
  • Document evaluations, progress notes, and discharge summaries accurately and timely in the EMR system.
  • Collaborate with interdisciplinary teams including physicians, nurses, physical therapists, and speech therapists to ensure coordinated patient care.
  • Educate patients and caregivers on home programs, safety strategies, and equipment use to support ongoing progress.
  • For Home Health, assesses patient need to recertify for Occupational Therapy. Prepares recertification Plan of Care and 60-day summary.
  • More duties as required.

You should have knowledge of and experience with:

  • Experience with orthopedic, neurological, and geriatric populations strongly preferred.
  • Prior experience in both home health and outpatient therapy is ideal, but not required.
  • Evidence-based occupational therapy interventions, adaptive equipment, and rehabilitation techniques. 
  • EMR documentation and therapy software.
  • Medication use and effects
  • Medicare, insurance, and regulatory guidelines for outpatient and home health OT. 

You have the ability to:

  • Work independently and manage a flexible schedule between clinic and home visits.
  • Strong time management and organizational skills to balance caseloads across multiple settings.
  • Proficiency in Microsoft Word, Excel, e-mail, and EMR systems. 
  • Excellent interpersonal skills with the ability to educate and motivate patients and caregivers.
  • Clinical judgment to make independent decisions and adapt interventions to patient needs.
