Physical Therapist Outpatient

Description: We need you as a Physical Therapist in our Outpatient orthopedic physical therapy clinic in Surprise, AZ.


To evaluate and treat a variety of orthopedic and sports medicine patients in an outpatient physical therapy facility. Requires a working knowledge of orthopedic physical therapy, pre and post operative care and progressive, evidence based treatment plans for all orthopedic diagnosis. This individual may be asked to participate in community education, staff in-services and volunteer with local high school sports programs.

Principal duties:
1. Manage 15-20 patients and their treatments per day
2. Instruction and direction of therapy techs in the set up and care of Patients in the specific tasks as directed by the physical therapist.
3. Charting daily treatment, initial evaluation, progress notes and discharge summaries utilizing electronic documentation system.
4. Follow up phone calls to patients and referring physicians as needed.
5. Correspondence as needed to referring physicians and third party payers in regards to patient treatment and progress.

Patients average age is in the 60's. We are 65% Medicare.

The facility is 3800 square feet. It is an open concept, with tables in the middle of the room and some against the walls with curtains if needed. We do have a gym area, staff lounge and separate offices for the clinicians.


As a well established and well known Physical Therapy clinic in our area we are concerned with the treatment of post-operative orthopedic procedures, fractures, acute sports injuries, arthritis, sprains, strains, back and neck pain, spinal conditions, and amputations.
