You will see patients in an around San Luis County. Your main office which we meet at is located in San Luis Obispo.
Under the direction of the Director of Patient Care Services and/or the Rehabilitation Supervisor, you'll develop and implement a plan for providing rehabilitation and home therapeutic treatment, as well as education of patients and family members. This position also coordinates with other home health staff to provide an efficient integration of care.
You will evaluate the patient's level of function by applying diagnostic procedures and develop/revise the physician's plan of treatment based on best practice principles and in accordance with all applicable licensure definitions and commonly accepted professional standards of practice.
You will also perform direct occupational therapy treatments based upon assessments and treatment plan.
A majority of patients are geriatric, however you may see pediatric or adult patients as well. Your orientation will be between 2 and 6 weeks depending on how much Home Health experience you have.
We provide you with a cell phone and tablet.
You will see, on average, 5-6 patients a day.
